Here you will find links to fill out your documentation on our protected and encrypted online patient portal, hosted by AcuSimple.  WE also believe and get if you're not super into tech and want to do it old skool and fill it out with paper and pen. Your choice! If you do prefer to do in person please arrive 15 minutes before your appointment time, this way your practitioner gets to spend your time with you!
